This repository is authored and maintained by a licensed physician practicing in Japan, certified as a Board-Certified Specialist in General Internal Medicine (Fellow of the Japanese Society of Internal Medicine).

Interdisciplinary Training: Prior to entering medical practice, the author completed a graduate-level curriculum and worked within private-sector corporate environments. The author later completed a competitive lateral-entry medical program (学士編入) and has accumulated 14 years of clinical experience as an internal medicine hospitalist as of 2026.

2. Context of Exposure to Primary Evidence

During the clinical events of August–September 2010, the author was a senior medical student with sufficient training to understand real-time physiological data and clinical workflows.

• Clinical Interpretation: The author was able to interpret bedside monitor data, laboratory trends, and procedural notes without relying solely on lay explanations.
• Decision-Making at the Time: Following the patient’s sudden deterioration during a CT session, the author requested an independent Judicial Autopsy (司法解剖) rather than an internal review.
• Long-Term Documentation Review: Using materials obtained through the court-ordered Evidence Preservation process (February 2011), the author conducted a long-term reconstruction of clinical datasets, imaging records, and administrative documents, as detailed in the Evidence Preservation Logs, the author reconstructed clinical datasets and administrative documents referenced in the Primary Evidence Index.
